The PersonalPlans Advisor Critical Illness Plan pays you a lump sum benefit when you are diagnosed with a covered illness. You choose the level of coverage—$X,000 to $Y,000. You can use the money as you like.
Covered conditions include:
- Heart attack
- Blindness
- Major organ failure
- End-stage kidney failure
- Occupational HIV
- Coronary bypass
- Benign brain tumor
- Coma
- Stroke
- Permanent paralysis
Your affordable group rate is determined once, by your age at the date of enrollment. After that, premiums remain the same, despite changes to your age, health, or individual claim. Premiums are conveniently deducted from your paycheck.
You can cover your spouse up to $XX,000, if you are covered. Your unmarried children through age 24 are automatically covered for XX percent of the employee benefit amount.
If you leave PersonalPlans Advisor, you may keep your coverage without having to answer any new health questions.
